Special Care is designed for people who usually do not ask for help themselves. There is often care-avoiding behavior with risk of pollution, nuisance, danger to themselves or others, risk of (domestic) violence and homelessness in combination with LVB problems. The request for help is often made by or through third parties. For example, the family doctor, the social neighborhood team or the Special Care Disclosure Office GGD. It usually involves multiple problems that are complex in nature and can pose a danger to both their own health and that of their loved ones. Think of honor-related problems, domestic violence, loverboys, childbearing and worrying pregnancies, nuisance, addiction problems, safety risk due to incipient crime and psychiatric problems.
The Special Care team of MEE Gelderse Poort supports the client, the network and the regular care network. They work together with the client's network, welfare and care partners, doctors, midwives, the security and justice chain, housing corporations and municipal facilities. Special Care's expertise is consultative, advisory or directorial. Insight into the quality of the social network is provided and it is ensured that the support process and communication fit the client. As a result, goals become achievable.
